Consult your healthcare provider before starting any medication. Your doctor or pharmacist should demonstrate the injection technique before your first self-injection.
Starting a GLP-1 medication like Ozempic (semaglutide) or Mounjaro (tirzepatide) is a significant step. For many Indian patients, self-injection is a new and anxiety-provoking experience. This step-by-step guide walks you through everything — from receiving your pen at the pharmacy to confidently giving your weekly injection.
| Item | Where to Buy | Approximate Cost |
|---|---|---|
| NovoFine Plus needles (4mm x 32G) | Medical stores near Ozempic pharmacies | Rs. 300–500 for 100 needles |
| Sharps disposal container | Medical supply stores | Rs. 100–200 |
| Alcohol swabs | Any chemist | Rs. 50–80 for a pack |
Look at the solution. It must be clear and colourless. Do not use if cloudy, discoloured, or if particles are visible. Check the expiry date printed on the pen.
Remove the pen from the refrigerator 30 minutes before injecting. Cold medication stings and is uncomfortable. Never inject directly from the fridge.
Wash with soap and water for at least 20 seconds. This is the single most important hygiene step.
GLP-1 injections are subcutaneous — into the fat just beneath the skin, not into a vein or muscle.
| Site | Notes |
|---|---|
| Abdomen (stomach) | Most commonly used. Avoid the 5cm area around the navel. |
| Front/outer thigh | Easy for self-injection. Pinch the skin. |
| Outer upper arm | Best for someone else to inject — harder to self-administer. |
Rotate sites every week. For example: left abdomen week 1, right abdomen week 2, left thigh week 3, right thigh week 4. Rotation prevents lipohypertrophy (hard fatty lumps that reduce absorption).
Avoid: bruised, scarred, or swollen skin; areas within 5cm of the navel; skin with visible veins or moles.
Step 1 — Prepare the pen Pull off the pen cap. Confirm the dose selector shows your prescribed dose (e.g., 0.25mg for the first four weeks). Screw a new NovoFine needle straight onto the pen tip. Remove both the outer and inner needle caps. Keep the outer cap — you will need it later.
Step 2 — Prime the pen (first use only) Turn the dose selector to the flow-check symbol (two lines). Hold the pen with the needle pointing upward. Tap the cartridge holder gently to bring air bubbles up. Press and hold the injection button until the dose counter shows "0" and a drop appears at the needle tip. The pen is now primed and ready.
Step 3 — Select your dose Turn the dose selector until your prescribed dose appears in the window (e.g., 0.25).
Step 4 — Clean the injection site Wipe the area with an alcohol swab. Wait 10 seconds for the skin to dry completely. Do not inject into wet skin — it stings significantly more.
Step 5 — Pinch and inject With your non-dominant hand, pinch a fold of skin (about 2–3 cm wide) at the injection site. Hold the pen like a pencil in your dominant hand. Insert the needle at a 90-degree angle (straight in). If you are very lean, use a 45-degree angle instead. Press the injection button slowly and firmly until the counter reaches "0."
Step 6 — Count to 6 After the counter reaches zero, count slowly to 6 before removing the needle. This ensures the complete dose is delivered and prevents the medication leaking back.
Step 7 — Remove and dispose Remove the needle while still pressing gently on the skin. Use the scoop method to recap the needle with the outer cap — never recap with your fingers (needle-stick risk). Unscrew and drop the needle into your sharps container. Replace the pen cap.
Step 8 — Do not rub Do not rub the injection site. A small redness or mark is normal and fades within minutes.
Mounjaro is simpler — single-use, no needle attachment needed.
Step 1 — Remove from fridge 30 minutes before. Do not shake.
Step 2 — Wash hands. Choose and clean the injection site.
Step 3 — Peel the foil tray open. Remove the pen. Do NOT remove the base cap until you are ready to inject.
Step 4 — Pull the base cap straight off.
Step 5 — Press the flat end firmly against the skin at 90 degrees.
Step 6 — Press and hold the orange button. You will hear a click. Continue pressing until the inspection window turns yellow — this confirms full dose delivery.
Step 7 — Lift the pen away. The needle retracts automatically. Dispose in a sharps container.
| Situation | Instructions |
|---|---|
| Unopened pen | Refrigerate at 2°C–8°C until expiry date |
| In-use Ozempic pen | Room temperature below 30°C for up to 6 weeks |
| Mounjaro pen | Single-use — use immediately after opening tray |
| Travel under 4 hours | Room temperature acceptable if below 30°C |
| Travel over 4 hours | Use an insulin cooler bag (Rs. 300–800 online) |
| Power cut | Below 30°C for up to 6 hours is acceptable; prolonged heat degrades medication |
| Freezing | NEVER freeze. A frozen pen must be discarded. |
"Will it hurt?" Most patients are surprised by how little pain there is. The 4mm, 32-gauge NovoFine needles used with Ozempic are extremely fine. Most patients describe it as "a mosquito bite" or nothing at all. If it hurts more than expected, verify you are injecting into fat (not muscle) and that the skin is dry before injection.
"What if I see a small bubble of medication on the skin after removing?" This is common and not a cause for alarm. If you counted to 6, the vast majority of the dose was delivered. A small surface drop does not mean the dose needs to be repeated.
"I missed my weekly injection day." If your next scheduled dose is more than 5 days away, take the missed dose as soon as you remember. If less than 5 days remain, skip the missed dose and resume your regular schedule. Never double-dose.
Do not throw loose needles in household bins. Used needles are classified as biomedical waste under India's Biomedical Waste Management Rules, 2016.
Q: Can a family member give me the injection? Yes. A spouse, family member, or nurse can give the injection. The outer upper arm is the easiest site for someone else to administer. Ensure they are trained in the same technique described above.
Q: Do I need separate needles for Mounjaro? No. Each Mounjaro auto-injector pen has a pre-attached, single-use, auto-retractable needle. Do not attempt to remove or reuse it.
Q: Can I inject on an empty stomach? Yes. Ozempic and Mounjaro (weekly injections) can be injected at any time regardless of meals. Rybelsus (oral semaglutide, taken daily) must be taken 30 minutes before the first food or drink of the day with plain water — swallow the tablet whole, do not crush or chew.
Consult your healthcare provider before starting any medication. Always have your doctor or pharmacist demonstrate the injection technique in person before your first self-injection.
